Placement of the Straight-In^TMSystem can be performed with an abdominal incision or laparoscopic entry. Access to the pelvic floor is created. 

First, the Y-graft is sutured to the vaginal cuff. Then, using the Straight-In inserter, the screws are placed into the sacrum. Finally, the Y-graft is secured to the sacrum using the screw sutures.

The sling is positioned to approximate a more normal patient anatomy.
