Step 1

Locate and make a midurethral vaginal incision of approximately 1.5 cm.

Step 2
Make two abdominal incisions 0.5-1 cm either side of the midline, just superior to pubic symphysis.

Step 3
Ensure the bladder is empty and position needle at suprapubic incision.

Step 4
Push needle through the rectus fascia.

Step 5
Move needle handle caphalad.

Step 6
Walk needle down posterior pubic bone through the retropubic space, exit vaginal incision and repeat on contralateral side.

Step 7
Perform cystoscopy, connect mesh to needles.

Step 8
Pull mesh up through suprapubic incisions.

Step 9
Cut away needles and connectors.

Step 10
Position the mesh under the mid-urethra without tension.

Step 11
Remove plastic sheaths, cut exposed mesh at skin incisions and close.

Step 12
Close vaginal incision.

The absorbable tensioning suture in the mesh can be used for further tensioning, if desired, after the plastic sheaths have been removed or in the immediate post-operative period.
